One page PHP/mysql script (with some design needed)

I have two mysql tables like this: Business business_id,business_name,logo,description,phone,address,latitude,longitude,website,category,hours_mon,hours_tues,hours_wed,hours_thurs,hours_fri,hours_sat,hours_sun,security_token Deals deal_id,business_id,time_created,type,description I need you to make me a page that allows me to ADD a business and 2 deals for that business on the same page. I will need these features: - logo needs to be a AJAX photouploader and AUTOMATICALLY crop/resize to 48px X 48px when submitted - when address is entered, it should use Googlemaps Autocomplete to suggest addresses (eg. [login to view URL]) and when complete the input it should set the latitude and longitude automatically - category is a dropdown (eg. food, travel) - hours_mon, ..., hours_sun should each be made of 2 dropdowns: OpenTime and CloseTime (eg. 05:00, 05:30, ...23:00) Below the business inputs, allow entering details for 2 deals. Deal 1 will be typeA, and have One inputbox (which goes into 'description'), Deal 2 will be typeB, have Two inputboxes (which goes into 'description) Finally a Submit button. When click on Submit, please validate for any empty inputs (All inputs must be completed including the deals). Then, insert the row into business table and the 2 rows into deal table, and show Success or Error at the top in a nice looking message, and allow the same process again. The page does Not need to be fancy, but it needs to be Professional. please start by giving me a mockup or html/css of what it looks like before coding.
