Webpage Design Work - repost

Design the patient page as shown in attached [login to view URL] patient table does not exist. I need you to design the page and patient table to be able to list the patients as shown. I want the page to have the SAME or similar soft look and feel as the images. The page is to be designed in php/mysql. The table only needs to display the sample data shown on the pdf from the mysql database. I will extend the database tables as needed for my future needs. I only need you to design the page and have the script retrieve the shown records from the table. I need the design work to be the same or very similar. That's it. The first coder with a decent demo will be awarded the project.
