Acrobat Form Development -- 2

I need 2 forms developed in Acrobat Forms with an option to convert the completed form to PDF and send to a particular email address. One of the forms is a 5 page (with normal spacing in Word) checklist and consent form type, with some check boxes and text boxes. This will also require an option to digitally sign and a send button which will export the form in to a PDF with a specific name format and send to an email address. The second form is a 2 page worksheet type form, with some text boxes (including one field to provide an email address) and a grid/table, e.g. list of tasks, their status and completion date etc. This also should have a send button which will send the file to a permanent email address as well as another email address provide in a text box. No signature requirement here. Since the document data is confidential, the form content will be shared only after the project is agreed and milestone is created. Please note that this is an urgent requirement and needs the task completed at the earliest. Best Regards, Jerome
