Create Proof of concept for passing an address, geocode address and determine polygons that point belongs to

Create a small proof of concept that consists of a webpage where:
1. Enter an address
2. Click enter and use a geocoding service to geocode the address (prefer to use a Google API for this)
3. For the point, return the polygons it belongs to.
Note: The shape / KML file(s) must be loaded into a mysql database and you must use the mysql database to determine the polygons that point belongs to.
The KML / Shape files will be provided to the winning bid. Please note there is only a single layer to be loaded into the mysql database.
Also, the layout of the example webpage is not important as it is only a proof of concept.
Note: must be completed in 3 days.
