App Developer Required

I require an app designing and developing to work on tablets. It is for building inspections so the inspector can create the report live on the tablet as he is proceeding through the inspection. instead of using a paper report and filling it in with a pen. It will have a cover page that will have company logo and other text that cannot be altered. it will need 4 fields (areas) where the inspector can and will enter information, like clients name, address, kind of report and date. The rest of the report will consist of predefined columns and rows, a total of 6 columns across the page, and rows of approximately 5mm down the full length of the page. it will need to print out onto an (A4) paper. In the first little box on the left it has its no: 1. Next box is category. Next Box Construction Condition. Then 2 small boxes these need to be developed to respond to the tablet pen as that is where the inspector indicates a pass or fail of a particular category. a pass he touches the first box and a green tick appears, but if it is a fail he touches the next box with the tablet pen and a red X appears indicating a fail, however this fail box when touched needs to expand and turn red allowing the inspector to type his fail findings in the expanded box?
