Node.js Back-End Fix for LP

We have built a landing page which requires a back-end part to be fixed. The scope of the work shouldn't be very hard to do since it has an example on other pages working stable. In fact all you need to do is to add back-end part from one page to another and make it work right.
What is the problem exactly:
The letters from this LP's forms looks wrong and don't contain all information we need there.
What we need to fix:
- Add time-out for sending letters
- Fix the letter's view and make it look similar to letters from other pages (titles, font formatting, UTM tags)
This job is one time project but we search a long term back-end developer for our Web-develop team. If this task will be done successfully, we are glad to work with you on other projects!
