I need to customize a file upload form to work on a bootstrap document

1. File upload:
I need a custom file upload that will display an input, a cancel button and a browse button.
When you click "browse" you'll select a file. The name of the file will be displayed in the input area. The cancel button will become active. Once pressed the cancel button will erase the file. Once you selected a file the Browse button will change to Upload and it will be selected.
