Hello, If I'm understanding correctly you need a form that can be accessed by a customer number you supply and the user will already have set up login username and id? Or will they just use that customer number as the login? Either way, I am confident I will be able to complete this project for you. I have 4 plus years of experience as a web developer with projects ranging from websites to web applications. I would be using PHP Laravel as a Framework in order to set all of this up, so I hope that php is acceptable. I would provide weekly updates until the project is completed in the 24 days. I use a project management method called SCRUM which provides you with updates and makes sure my timeline is kept. In projects like these, it is important that we keep a pretty open line of communication, so we will either need to set up daily emails or messages in order to keep to the project deadline as well.
