Deign a quiz website/database

I'd like a website created that generates math quizzes consisting of 10 questions. Quizzes should be randomly generated from a bank of questions with specific criteria. We need bank of questions which I will later create. These questions will be graded (easy, medium, hard). The user must be able to select a quiz difficulty (easy, medium, hard). For example, if they select easy: The first 7 questions should come from the easy bank. The next 2 questions from the medium bank. The final question from the hard bank. The problems will be text based with values that are assigned from a table ie, there may be 10 variations of the same question with different values, for example: "John has x apples, if he picks another y apples how many does he have?" The questions should be able to be displayed one at a time, on the same page (line by line). Once all questions have been displayed, answers should be revealed next to the question one at a time. A decent example of this is "[login to view URL]" I will need the ability to add and modify questions after the project is finished. Please allow some time to explain how it has been created and to show me how. I really want a quality product. There will likely be follow-up work as a result.
