custom excel spreadsheet

Looking for a fill in the blanks type of spreadsheet. I do school and little league photography and I take care of several teams when i photograph them. Instead of coming back to my office to hand write each customer name, For Example: ABC League ( at top of spreadsheet to name the league) I need the year too. Next line, I need Frame #(1-500)so it can go in shoot order)). Next line, I need customer first and last name(for the ones who order from me). Next line, I need an area for each package they order. I have 24 packages (A-U and a Game Day Special) Prices are different for each package. I would like to be able to do the accounting for each league/project i photograph at the end of the photo shoot. I give a percentage of the sales to the leagues as a kickback. I will need to have this added in as well. At the very end of typing in the job i would love to be able to have 3 sheets to print. 1 for studio showing every expense for the job(hotel, gas, food, misc) 1 for the league showing who ordered and what they ordered but not showing any expenses just the percentage they get(10-40%). I have uploaded a PDF of the job and amount of expenses.
