Please note the features i am looking for in this. - It should work on mobile and web - It will have a form which will have the user input his or her details. These details should be captured and i should be able to access them later in the form of google or excel sheet on my email or gdrive. - It will have interactive surveys (with custom images - which i shall provide) - responses of which should be recorded and be available to me later for review. - It will have playable video .
