The template theme we already have is a Medicenter. Color to use Teal. This Start up company needs 4 main pages now. The VIEW the MEDICENTER Template pages we want are HOME (Style 2), View Pages drop down (ABOUT US and Medical Care page will be replaced word for SERVICES) and CONTACT page. -HOME Page , There are 2 Hectagon Shape already there, we need 3,which will have next to the shape: Personal Assistance Companionship Home Helper -Each of these will link to the Services page. Above these categories put "Our Services" -On Home page Remove Our Clinic. -Make sure all the links work and go to the pages they should go to. *Put LOGO on pages and bottom Copyright...with company name. ABOUT page: REMOVE Meet the Team. Put a 3rd Hectagon Shape SERVICES (Medical Care) page, See below where has Laboratory that shows how want the 3 Hectagon Shape on pages specified. CONTACT page (Remove the Map) Box put on RIGHT for Register and BOX for BECOME a CAREGIVER link to our WuFoo Form (We have a Wufoo Form) The form will need to add some check box for services needed. We have image/text ready to put on pages. The site should be upload on our [login to view URL] server and should work, all links for final release of payment. **Please send a mock or and sample of your previous work completed.
