We need someone to design the following for our website: - a form for applicants to submit their resume to us (needs to be stored in DB) field s to be included (all required fields): - First and last name - address (street, city, country) - phone number - email, availability (date) - project duration (to select from "up to 2 weeks, 1 - 3 months, 6-12 months, 12+ months" - multiple selection possible) - salary required in $ (per day) - geographical location (to select from" local, in home country, US, UK, Europe, Middle East, APAC, LATAM, worldwide, Remote, Remote with short term at client site" - multiple selection possible) - Work/residency permits (text field) - Main Skills (to select from IT Management, IT Consulting, IT Security, IT Project Management, IT security administration - multiple selection possible) - attaching CV in Word or PDF format - Security clearance (yes, no) - a DB backend/management site online (accessible for web admins with password only) with search functionality (see above) - notification via email once a candidate sends us CV (Name and attached CV is enough) Will work with coder to provide access to website. Coder needs to set up page and DB (Hostgator) and provide testing. Coder needs to be available for later followup if DB needs to be expanded or modified later on (will be separate project).
