Blend your own tea

I have a shopify website, [login to view URL], where I sell organic loose leaf tea. I would like to offer the customers a unique opportunity to create their own tea blends by providing a simple page form where they can select a number of different ingredients from drop down menus. Once they have selected up to four ingredients, and press enter, the price will add up and get entered into the shopping cart. To summarise, what I am looking to have done is the skeleton form, the ability to calculate the cost of the choosen ingredients and communicate with the shopify cart by entering the product summary and cost into the cart. For example a form like (I can enter in options later): Step One – choose your base tea: option1, option2, option3.... Step Two – choose your first flavour: a. Floral – option1,option2,option3.... etc b. Herbs - option1,option2,option3.... etc c. Fruity - option1,option2,option3.... etc d. Health - option1,option2,option3.... etc Step Three – choose your second flavour: a. Floral – option1,option2,option3.... etc b. Herbs - option1,option2,option3.... etc c. Fruity - option1,option2,option3.... etc d. Health - option1,option2,option3.... etc Step Four – choose your third flavour: a. Floral – option1,option2,option3.... etc b. Herbs - option1,option2,option3.... etc c. Fruity - option1,option2,option3.... etc d. Health - option1,option2,option3.... etc Step Five – Enter any special requests Finished! And the cost will calculate and get entered as a product in the shopping cart.
