Design of auto accident website

Hi, We need a design of a website. The business is based on a toll free number that a person can call when they are injured by auto/slip and fall/ or any type of injury. We are OK if you use a template but it must have a look and feel of what the business model is based on. We will supply the logo and content, you will be responsible for look and feel of site including having a contact form and displaying videos (which we supply to you). Thanks
