Custom Database required

I have a requirement for an easy to use CRM type system I have uploaded a rough excel template to give an idea. I am looking for a web2 style interface with large clear buttons and control panel type interface for links etc. There will be user roles, once the database is complete I will confirm what sections need locking down. The database will hold customer information and manage the flow from site survey to order and support The app will need to output to word or pdf templates and these will also need designing. The application needs to link into Exchange 2010 for a resource planner. The application is to be hosted locally and will not be accessed over the internet. All source code and SQL must be available and clearly commented. Will require a user guide and will need to make minor changes during the user testing phase.
