Custom PHP file

Hi, this is a very easy work for a php expert, i have a newsletter in flash and php made, when someone fills the form, then sends all the information to a database, and sents a email to the user to make a conformation, well i just want to custom this php file, to add a header image in top and a button on right, thats all! Just add code to show image on top, and image on right side, easy job, i have 10 dlls in my account ready for someone who can help me on this asap,i have the php file ready , send me message on PM, thanks !
