Apply one webpage to site, using XHTML/CSS(repost)

I have an existing webpage that needs to be applied to the homepage of a PHP/MySQL site. The existing page is written in table-based HTML, but MUST use the new site's XHTML and existing CSS elements (or create new CSS elements within the existing style sheet if need be). Solution must follow existing code structure of the PHP site. The new page includes a header, a footer, and a content area--so the existing page must be put into this format. I will check this, so please be sure your PHP skills are adequate to apply logon code, etc., that will be part of the job. PLEASE SEE THE ATTACHED DOC FOR SITE INFORMATION AND THE EXACT PAGE THAT NEEDS TO BE APPLIED. AGAIN: HTML WORK MUST BE CODED WITH XHTML AND CSS IN EXISTING TEMPLATE FORMAT. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
