ajax/php designed contact form

1.) What I need is a perfect new look and function for my contact form on my website. (The image I overlayed is a little too big, so you have to make the contact form fit in that space of course) -> Please take a look at the pic: text [login to view URL] - Here I wrote every sentence and word that has to be changed and 2 lines of the contact form has to be changed, this is highlighted in blue in that pic. The pic shows the contact form of [login to view URL] - you can make it in that style and color also. Don't forget the function, like highlighted box when clicking on one, and the other coding to make a contact form work... My site is in php so it should work in php. The second part of the job is make a simple table in html look better or even fancier. The table is here: [login to view URL] -> Please make it in photoshop or with a good design software, so the result is one image! Please overtake all words and numbers as you see in the table. Shortly: Pimp this table! ---------- The worksheet gives you everything you need to start right away. As I already worked several times with freelancers on GAF, I made many bad experencies in quality of work. So please only bid if you are really aware of completing this job perfectly - that means desgning and contact form coding! Max. I agree to pay is USD 80! Show me some examples of your work...
