Develop downloadable PDF Joomla component for form records

We have built a multi-page online form using Breezing Forms in Joomla. This form collects the results and stores the info in the Joomla DB. Our client would like to be able to login to Joomla to retrieve the records/submissions of this form in a nice looking printable PDF format that would print to be used in their office for the patient's to sign when they arrive. The PDF would be a single form submission laid out like a office form, NOT a listing of all the records. The records would be organized by name and date for easy retrieval. There would be an existing static pdf to follow for the basis for the design of the downloadable version, although we would like to be cleaned up and look nicer. There is a component called Logos ([login to view URL]) for Joomla that may be the best solution for this, but I am open to suggestions. Please message me with your name and examples of your work.
