Design and Create Online Diabetes Logbook - repost

This is a proof of concept website for a medical practice. Create an online diabetes logbook for patients to record their blood sugar values. The online diabetes log will allow pregnant patients with gestational diabetes to log in and record their blood sugar values. Patients should also have the ability to record their medications and the food they consume. Administrators (doctors in the practice) should be able to log into a central hub and review the patients blood sugar logs. Ideally, the patients should have the ability to print out copies of their blood sugar values or send them by fax or email to another doctor or a family member. This website will be similar in design and function to [login to view URL] OR [login to view URL] This website is strictly for proof of concept. If our practice decides that it is a workable solution we would then incorporate it into our current website. Eventually, we would be interested in allowing patients to enter information into the site by SMS (perhaps using an API like Twilio ?). We have a number of other plans for this site (blood pressure monitoring, weight, medication tracking, etc). So if this proof of concept works it could become an ongoing project.
