Create Optin Form For Website (Aweber Code provided) - Repos

I am looking for a reliable website coder and designer. My first project is to create a more appealing optin form than the one currently on my site. I already have the Aweber code to collect the name and email and what I want is this integrated with a snazzy looking optin box that will appeal to people to sign up for a free 1 day trial. Once you have done the design and I am happy with this it will then need to be uploaded into the site. I am looking for someone with a view to other long term projects that I have.
