Build a simple table editor

We need someone to create a very simple web-based data editor for some existing data in a mysql table. Multiple users should be able to edit records, but not the same record concurrently. The application essentially has two 'views', one for the list of records (with search function), paginated, and the edit form. The other view is the record edit form. If a record is selected for editing, that has one field (i.e. the phone number) the same as some other records, then multiple forms (one for each record) should be displayed in the edit view. The user login data (for accessing the application) is stored in a separate table. Database structure and data already exist.
