Data Input to create a MS Word Document - DB Back End Required.

This software will record details taken during a telephone interview and insert the data into a MS Word document. The nature of the interview is to record statements of complaints made to local area housing officials with regard to housing disrepair claims. Much of the interviews are a series of fact finding. However, the purpose of the software is to eliminate as much of the typing as possible, and replace this with mouse clicks via drop down menus etc. Part 1: Please see "instruction1" attached. Details to be included: 1. Name, address and DOB of claimant, 2. Name of County Court (taken from the district section of claimant's address) 3. Landlord and address (database back end) 4. Local Area Office (filtered database from Landlord)5. Weekly rent (amount) and who pays. 6 Surveyor (database back end) Part 2: Please see "instruction2" attached (preliminary guidance only - detailed discussion required but will follow this format) 1 - When the defect became apparent. 2 - When reported. 3 - Who the defect was reported to. 4 - Method of reporting. 5 - Response from complaint. 6 - What happened after the response. (whatever happened the result would be unsatisfactory) 7 - Was the defect reported again, if yes back to 2, if no then onto Part 3 Part 3: Impact of defect (differing impact for each type of defect) These are just guidelines and more detailed discussion is required. 1 - (defect = damp wall) effect may be any or all of the following: Unsightly, Ruined decor, Smelly, Embarrassing, Black Mould, Salt precipitation etc etc etc 2 - (defect = ill fitting window frame) effect may be Draughty, Cold, Higher Heating Bills, Uncomfortable, Rain Penetration, damaged decor, etc etc etc Much more time will be given to part 3 as close communication will be required. I am aware that the software cannot be artificially intelligent, but the purpose is to provide a framework to work after having finished the telephone interview. Thanks.

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Complete ownership and distribution copyrights to all work purchased.
