Competition Scorer

I am currently working on my IB higher level computer science school project. My project is basically a PHP webpage that allows people to insert the times of recorded events during a school sporting event, such as swimming gala or sports day. The criterion A attachment is a description of what my project aims to do, who it is for, and how I intend to fix this problem that my school is having. This does not have to be a complicated project, and doesn't have to be marketable. It just has to be a simple web design (as I have drawn in the 3 screen shots) , and match my design (criterion b). I need help writing code to create a MySql database, and be able to get this data on a PHP webpage. I hope that this is enough information for anyone who is willing to help!
