Clone of Widget Creation Tool

I need an entry page for the Event type as listed on their site: Title, Date, Repeat, Event URL I will host this on [login to view URL] and want the output to be ad-free (no reference back to [login to view URL]) AND want the parameters of the input form in the two output boxes for iframe and ahref as similar to [login to view URL] Thank you.
