Complete pHp/javascript code for a file upload page - bootstrap

Hi, I am writing some code for a bootstrap based web application and require some help to complete one page. The code is mostly complete, but is not functioning correctly. You will have to review the current code, and adjust it to make it work as described below. Some expertise in php and jquery will be required, and preferable with some experience with bootstrap for the modals. 1) Open the page [login to view URL], you may also require server/php/index.php. 2) I want the the top widget (upload files) in a bootstrap modal called #uploadModal (This is called up by user clicking on 'Add Images' button). I have done this, but the code in the modal does not work. Please fix this so that it works as per the widget. Then do the following to the modal... 3) When files are uploaded, they are uploaded to server/php/files/{%[login to view URL]%}/{%[login to view URL]%} and also a thumbnail is created in server/php/files/{%[login to view URL]%}/thumbnail/{%[login to view URL]%}. I also want the file location saved to a MYSQL database. When the user clicks on the 'Submit' button, I need to carry through the inputs 'entry' and 'file_path' so that these are saved to the database by the AddQuery at the top of the page. The database and tables is already made (d_gallery and d_gallery_files). 4) Fix up these AddQuery commands (lines 29 and 34). They should save the diary entry (text) and the file path to all the uploaded images. The d_gallery table is created first. This holds a column gallery_id that is required in the d_gallery_files table where the file paths are stored. Therefore, an AJAX script of something similar will be required to enable this. 5) The images shown on the Photo Gallery Widget are the large versions. Please change this to show the thumbnail verisons instead. 6) The larger images should be shown in the modal 'myModal' that is called up by clicking on the thumbnail. Please fix this. Also, the Previous and Next Buttons in myModal should bring up the next image on the photo gallery into myModal. 7) The Photo Gallery Widget needs to be formatted so that images are displayed in rows of 6. After six images a new row should commence. If you have any questions, please ask. I will be online during to enable you to discuss this directly with me while the work is being carried out.
