Create a simpl form in php having 7 fields

Hello, Need a form for our site. A simple form which can be quickly seen on the site with a click of a button. On clicking button I need 8 fileds. Below the form I need a submit button, reset button and Add Another Requests button. It is must to fill the form first in order to fill more than one requests. On clicking add another requests button, the requests above in the form gets shifted to a table with rows and columns. Similarly table can accept as many requests as possible and once filled, the whole table should be able to sent to the email on submit. I will send you the fileds whosoever gets selected.
