Custom Form in Homepage Slider (Modern, i.e. No Flash)

Project Summary: I have a wordpress site that I would like to implement a 3-step form within the homepage image slider. There is no slider at the time, just a static image. So, the awarded freelancer needs to integrate the 3-step form and the slider into the existing Wordpress theme* framework. *We're using a free WP theme called "Elegant," you can find it here in case you want to practice: [login to view URL] Project detail: The homepage slider will actually not be a standard image slider, rather it will be a designed and stylized box (PSD provided) that contains interactive forms. There is a 3-step process: 1) User selects a service from our drop down menu on the first form; 2) then a new box of the same size slides in from the right and offers the user a new form where they enter their contact information; 3) then a new box of the same size slides in from the right again and offers a Success type of message. Notes: 1. The administrator will receive just ONE email, logging the result of Step 1 (Service choice) and Step 2 (Contact info) 2. The "Success" step must contain a Google Conversion Code so we can track the form's usability/success 3. You may use any technology you like, we want this to look fresh and clean and cannot risk having the code break/not display correctly. Technologies we have in mind include JQuery, Ajax, HTML5, CSS. We cannot use Flash because of iDevice incompatibility. 4. We have a PSD of the design, but the design elements are flexible. 5. This code/design will go on a website which is a long-term project, if you give us value here, we'll need your services in the near future.
