Create web form to write data directly to Opportunity Record in infusionsoft

We need a developer to create a webform that will write data directly to a customer's opportunity record fields within Infusionsoft CRM. Essential - developer MUST be fully conversant with Infusionsoft CRM and its API/SDK. ## Deliverables The developer will have exstensive knowledge of Infusionsoft CRM, Infusionsoft API and integration with simple webform on a Linux server platform. Code will be done in PhP. API will be provided and SDK if required. The form will be a simple web form where the user will type in the person's email address. The form will pull the customer's name and telephone numbers from Infusionsoft. The user will have access to the current list of opportunity record Stage ID's in their drop down menu on the web form. After doing a search for the user and the form brings up the user's phone numbers available for the customer, the user will fill information on a text area field and when they click the submit button, the form will have a time and date-stamp that will be seen in the opportunity record in Infusionsoft. The user will also be able to select from the Stage ID dropdown option. After the user completes the form - the user be able to see a screen dump of that they have written just in case they want to print it (we should have the option to switch off printing on that page if we want to as an admin function). The winning bidder will get all the required field names - mind you since this is a simple call logging form there won't be many fields.
