Client Side Javascript Calculation Tool

I am looking for someone to? develop? a self contained html/javascript application that will do the following: *Show the user a simple form asking for a mode of travel *Based on mode either use an interactive google map distance calculator (url supplied) or a simple mileage entry form field *Ask user number of hotel nights simple form field Multiply supplied values by a factor and display a table of the results and a pie chart (use a javascript library of your choice) That's it.? ## Deliverables <col width="75" /> <col width="154" /> <col width="75" span="2" /> <col width="224" /> Here is an outline of the form as I see it Mode of Travel Selection box between (airplane, automobile train and bus) If user selects Airplane then show the following interface: <col width="75" /> <col width="154" /> <col width="75" span="2" /> <col width="224" /> Please use the map below to enter your flight intinarary from start to finnish in the order you flew This should be implemented using the calculator at this location [login to view URL] If the User choose another mode then the map should not be displayed and instead we should have a simple entry box miles. Estimated distance travelled: ? The rest of the form; Number of Nights Hotel Nights: A summary table is then displayed with a pie chart besides it (uses whatever javasscript library you want) Actvity ? Total Travel ? ? (if airplane = miles*3, if auto= miles *4, if bus = miles* 5, if train = miles* 6) Hotel ? ? ? Hotel Nights*43 Total ? ? ? Sum of travel plus hotel Pie chart of travel and hotel goes besides the table (see attache file for details on the form) There is to be no submit button the calculation and values should change in a "live" fashion all calculations are too be on the client side
