Design and create landing page (with flash) & thank you page

What we are doing here is creating a landing webpage. We are testing if we can get people who come to a page to fill out one of three forms on the right hand side, and seeing which one they fill out. * What I need is for you to create 2 Web pages. Firstly the landing page - This involves? some flash and creating a? "contact us"? form that auto generates an email with the information filled out. * A? simple “thank you for filling out the Form?? page which has almost nothing on it. It muxt be done PHP. Please see attached document for full details. I have provided a good description so the job is clear.
