My customer does health screening for large employers.
They need to produce a report for their customers showing health statistics for the employees they have screened and a measurement of how the statistics have changed since the previous screening - reporting any new cases of a problem and also counting how many are the same, how many are better, how many are worse for each statistic.
I have an Access database containing 4 or 5 tables I have imported from their main system, you will build the report against this database.
Create a screen for the user to select company(s), business division(s), site location(s), date range of screenings.
The current and previous screening for an employee might not have the same client code, you will have to do some work trying to find the previous record for the current screening - probably looking at last name, date of birth, gender, etc. This will almost certainly need to be done with VB code instead of an Access query.
The data is very confidential, after I award the project I will have to send you the data tables with the names and addresses scrambled up.
The field names in the data tables aren't meaningful so my reports will show you which fields contain the data you need.
You will produce the detailed report in tabular format in MS Excel 2000 so that the customer can edit the report data when required. You will produce the summary report in MS Word 2000. The totals in the Word summary report will agree with the totals at the bottom of the Excdel tabular report.

Access database with no data (I can't upload the separate data database - too confidential), look at:
qryReport, this shows where some of the statistics come from in the tables,
qryJoined shows the relationships between the tables.
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
