Access Database project

I have a very small database project I need to be done in Microsoft Access. I will need the coder to. 1. Design the database schema(very simple) 2. Design the forms(No more then 4) 3. Design the necessary reports(Need the reports to do calculations) Somewhat complex. The most difficult part of the project. Description of database. A "grid" with a series of numbers about each person in the database that the end user can enter a client number in the form and find. The grid contains information about each test that a user has taken. Each test receives its own row. The clients need to be able to be marked "active" and "inactive". The client data will include Name Client # Scores for tests. Five numbers for each test The Reports will include things like: Example: Totals, means, medians for test 1,2,3 for all active clients. The Forms wil lbe for entering the test scores and manageing what clients are active and inactive. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Access 2003 Windows XP
