I need a form created that validates and sends an email to both the customer and the client showing the form details. The form needs to look exactly like the image supplied, Simple job for someone with the right skills, has to validate all 5 fields and needs to have a anti spam code. This code is php and should use phpmailer to send the emails I can provide sample code for anti spam code and email templates, plus photoshop with layers fonts etc
