Create forms to add data to database

Database tables provided, This job is the creation of two forms for adding details to a MySql database.
This needs to include the ability to add edit and delete records, with form validation. These forms will include the ability to upload photos, and pdf's.
The two things that are added are restaurant details, and then deals for restaurants, From the form to add the deals you need to be able to add restaurants that will have this deal, so the deal form will have a list of all the restaurants and checkboxes to link them the deal, this linking is then done through a
linking table which is provided. The forms need to match the database tables provided. Images and menu uploads are to be placed in a folder to be retreived.
Once these two forms are complete, I need the database records to be retrieved and placed in pages that are provided, showing restaurant details and deals.
This is a very simple and quick job for someone with good php and mysql skills
I need the entries to appear in a list table, which you can click through to edit the deals and restaurants, the ability to search the listings is also important for both restaurants and deals
