CAC Survey

I need to have one ASP page created to display a survey that looks like the attached word document. 8 questions with check boxes and fill in the blanks. After the user fills in the survey fields, they would click Submit and the survey results should be formatted and sent via email using CDONTS to an email address. The results in the email should be in the order as they are displayed on the page in an easy to read format. The user should see a message that says "Thanks for taking the survey". Thanks.
