Adding multiple file upluad and mailing to a HTML form

I have done a mini form which has fields like name-surname,date of birth, adress, phone etc... I want to add some features to this form like: 1 - Multiple file upload element where the people can add or remove files. This must be done with ajax or jquery. 2 - A submit button that will send the all form datas and the uploaded files (as an attachment) to a given e-mail address. 3 - And at the end a thanks message to the user.
