I need somebody to build an interactive Camera Report Sheet for use on Ipad. The files need to be saved in order to be emailed to the production office. I have attached an example of a traditional camera report sheet. The fields which need to be interactive are as follows:

- ROLL# - Production Info

- Production Name: - Date

- Director:

- DOP: - CAM: - Operator:

SCENE - SHOT - TAKE - ESSENTIAL INFO

Under ESSENTIAL INFO There need to be 3 selectable and 1 input field.

Lens - Primes: 6, 8, 10, 12, 14, 16, 18, 25, 32, 35, 40, 50, 60, 75, 85, 100, 135, 150, 200, 300, 600mm

- Zooms: 16 - 42, 30 - 80, 17 - 80, 18 - 100, 24 - 290, 25 - 250, 150 - 600mm

T-Stop - T1.3, T1.4, T1.4 1/3, T1.4 1/2, T1.4 etc

Filters : ND3, ND6, ND9 etc

Input field: for focus and additional details.
