Apple Iphone/Ipad Contest App

We want an app where people can enter a contest through the app in order to win a watch. We want to have people put in there information to be entered in the monthly contest. We want them to have to enter their E-mail address and names with optional fields for address and phone number. We want to have this App promoted so that people around the United States can enter and so that our business can gain popularity. All while collecting information of potential customers.
