Contact Form and Modify Graphic

Hello,

We need a contact form to be added to this page:

Currently the page is updated with a custom CMS but the CMS does not have the functionality for a contact form. We need the contact form to allow us to add or remove extra fields in the future (with some fields being required and some fields being optional). The form in the meantime should have the following fields:

We should be able to edit the form from the current CMS and without touching code.

The second thing we need fixed is simply cosmetic. In the current CMS we can have a PDF that is available for download on different pages as can be seen here:

This is the gold box section to the right. We need this to be much more attractive and it needs to be clear to people when the information was sent, at the moment a person doesn't know if the form worked after clicking send.

Lastly we want this same investor report to be available at the bottom of each page as can be seen here:
