Develop a wordpress plugin

Hi, We want to have a form which signed up WP members (using wp-members) can fill out and send. The form should have the look and feel as well as the functionalities from [login to view URL] However we do not need a builder - the form can be hard coded. We will send an example form to see the uses question types. ([login to view URL] but this is not finished yet, and the css has to be adaped to the currently used at [login to view URL]) We want to include a very basic calculation - if the user enters 2 or 3 numbers in different fields a calculation sum should be displayed at the end of the form. The submitted form should be stored either in the WP Database or in a google spreadsheet. The form has to transmit the user credentials as well as all filled out fields (including a file upload) Optional: We want to give the user the opportunity to view his submitted forms and we can provide a status code. Project language is english / the form should be then in german but we will provide the translations.
