Design a call to action button and code to html

We are looking for a designer to create a better call to action button for us. We have created some web forms ourselves and realised that our call to action button is rubbish. As it is probably the most important part of the form we are best to get some help. The idea of our business is to deliver tenants a free moving quote service. We have already captured the tenants details through our partner and require their move in date so we can pass that onto our movers (for a small fee). Our form recognises they have opted in for our service and they need to provide their move in date. The current (rubbish) call to action button needs to be upgraded ASAP. We require the design and the html code for this button so we can pass on the package to our web guys. I have attached the current button and the form to hopefully give you some context.
