Design my book cover - repost 2

I need a book cover designed that will be used for a print book and ebook. I have an image attached to use that can be updated or altered to incorporate into book cover. The front book cover image in the end MUST be 6.25 x 9.5 (WIDTH AND HEIGHT) and 300 dpi. The following text must be incorporated on the book cover which is the author and title of the book.
