Hi, I would start from scratch since a form like that is easy to create quickly. And as you make it yourself you have more control over the layout - although its inside an existing website. The problem you're facing with the form is the processing of the information. I assume your expecting an email with the information, but since sending information thought a server and email can be interupted you will want a backup in a mysql database, incase you miss any information. The best way to go would be to have a back-end facility to have a total overview of your request - so you can do follow up on them. But this is not included in my offer.
