Build a wordpress page for new applicants

I need a page designed for my website that is similar to this page: [login to view URL] Basically it is an online job application form. I'd need to be able to change the form questions when I need to and to customize the text as I need. I would also need to have the results emailed to my email address each time a person applies on that page. My current page is [login to view URL] Please show me some examples of relevant work.
