After my flash video plays, replace it w/ my zip start form

I have a flash video on a webpage. After the video plays, I want it to be replaced with my zip code start form (already coded on my other site). I don't want the form implemented as a popup or redirect. I want the actual video on the page to be replaced with my form. I can paste the code and modify it you can provide some kind of code boilerplate and tell me what must be done. If replacing the video is not possible, another acceptable solution is to have my start form appear somewhere else on the page (e.g., to the side or bottom of the video) AFTER the video plays. If you bid, please let me know which solution you can help me implement. Thanks!
