Do some Excel Work

Guesstimate 6 hours work I have a job that requires me to keep a logbook book of my activity. Until recently I had an iphone app that recorded the info and produced a standard output form in a specified format. Sadly with the new iOS the app no longer works. The good news is I managed to take all of my data from the app in a CSV and have the data in an excel spreadsheet now. I have been using this spreadsheet to add activity recently. So now I need to produce a report and although I have a good grasp of excel whatever I make will be clunky and take hours and hours. What I would like to do is have someone who is familiar with excel knock it up in a 10th of the time. Rather than be a poorly specified project and be a pain to do this should be relatively easy as: 1. I have sample data in the exact format of the excel file I have 2. I can supply the output file so there is a fixed endpoint for you to meet. 3. I can reply promptly to queries 4. I can even video conf you if you like Some questions you may have: A)The fields for "surgical speciality" "procedures" "primary anaesthetic" "secondary anaesthetic" must be populated by looking at what instances exist. i.e. If I was to add another procedure type in future the report would need to be able to include it and then generate the numbers related to it. B)I have a much bigger data set than the test set sent so whatever you make and however you make it I would need to be able to apply it to a different data set (all the columns are in the same order C)Output type. Pref RTF document please. Grossly in the style of the attached report D) The xls file has a tab where I can enter my name and grade and employer and also a date range. The report needs to be based on this DATE RANGE i.e. not just on all the entries in the spreadsheet Thanks for your time C
