The "Update DB" macro of the attached spreadsheet needs a minor update. If you enter data and click "Submit Data" it seems to work fine. There is a problem when you delete the current (dummy) data and start entering new info in the "Checklist data" tab. When you submit new data from scratch it appears in row 6 of the "Data" tab instead of row 5. The issue is in line 14 of the macro. Specifically the formula for "LR". It needs to be updated so it works for a blank sheet. Should be a simple problem to fix for someone with VBA experience. Will pay £20 rushing charge if complete within 12 hours.
