Hi, I need two simple templates made for my staff to log their entries and exit on a day to day basis. They are both similar and only the main body and title changes between the two.

It should:

1. Use Calibri as a font
2. Have the congo store logo on the top and the other logos in small somewhere (top as well or bottom even)
3. Follow colour schemes of Congo Store

The template needs the following details to be captured on the top:

1. Site Name
2. Serial No
3. Guard Incharge
4. Mobile Number
5. Week Beginning

The title is Staff Entry-Exit Log

The main body (columns should be like a list)

1. Line No
2. Date
2. ID NO
3. Name
4. Time Out
5. Signature
6. Time in
7. Signature
8. Reason

On the bottom it must have a box for:

1. Guard Signature (And date submitted)
2. Accounts Signature and date checked
3. Directors signature and date approved


For the second one:

The title is Site Entry-Exit Log

The main body (columns should be like a list)

1. Line No
2. Date
2. ID No
3. Name
4. Time in
5. Time out
6. Signature

everything else same as the first template.

Looking for something simple and looks clean.
